One episode in season one called My Old Lady focuses on the plot of JD carring for an elderly woman who has a DNR ( do not recessitate request) it is a very emotionally intense episode and it may be difficult to watch if you have a loved one who is aging or has terminal illness.
Scrubs is not graphic in any way but some people who find surgery disturbing might be unsettled by some scenes.
As this series is set in a hospital, plenty of patients die, suffer, or have a terminal disease, and certain episodes are very sad and depressing.
