Woman gets killed offscreen by the jack in the box. You see scratch Mark's and dents in her skin before being dragged into the box.

Multiple men and women are killed by the jack. You see blood, but none of the killings are remotely realistic.

Movie contains some scenes of violence and some aberrant scenes that may disturb children. Movie has a rough NC-17 rating really deserving it. Its should be an r rating.
