Hannibal Lecter opens the skull on a man, while he is still alive and fully conscious, and feeds him his own brain. The top of the mans head is graphically cut off and the brain is fully exposed and very gory for an extended period of time. Hannibal graphically removes the brain sac and slices a piece off (the actual slicing of the brain itself is never shown, but many people mistakenly remember having seen it after watching the film), which he fries. The man then happily eats his brain as blood runs down his face. At the film's conclusion Hannibal feeds some of the cooked brain matter to a young boy. Very explicit and disturbing.
