An ashen figure slithers down a flight of stairs and toward a teenage girl, the ashen hands grab the girl's sweatshirt, it pulls the girl down and she disappears; a boy holds the sweatshirt up and is frightened by what he sees.

A young man develops a photograph in a darkroom, a spirit comes out of the photograph and kills him (he is later shown slumped against a wall when a young woman finds him).

A woman restrained to a hospital bed is frightened when lights in her room flicker; an ashen hand reaches up from beneath the bed, touches the woman's hand, and the woman struggles to free herself. An ashen head then rises up from behind the bed, the woman frees herself and runs through the hospital; she turns around to see the lights in the hallway turning off and a shadowy image of an ashen and twisted woman lumbering toward her.

The developing room for photographs where Kayako emerges from a photo submerged in water is genuinely creepy and frightening.

Although not gory, the scene where Kayako drags Karen off the hospital roof and kills her is genuinely disturbing, as Karen's blood splatters all over Aubrey and Eason, both of whom are visibly shocked.

The scene where a possessed Sally drinks and regurgitates a gallon of milk may be nauseating to some viewers.

There is some imagery of Kayako's face and body, she cracks and flops when she walks in addition to some breathing effects. This may be disturbing for some viewers.
